iii.  His  Heihgt / Body weight / Girth  of  chest and  abdomen ( Naval -level )
IV. Disease their  parent  are  now  suffering  or suffered  before  their Death ( God  forbid !)
V. Any  hereditary   disease / addicting  heritage in Family.
